Maybe the IC was inserted the wrong way around or a solder blob... bent pin etc.
The pin gap from the package MUST be removed from the socket to.
Is removed because of high voltage isolation necessary there and more, with pin removed is easy to not insert the IC wrong.
It's not a good idea to socket PWM-FET ICs, because they use the Source/return and Drain leads for keeping the IC cool. In a socket, this won't work and the IC will overheat.
