Gigabyte RX 580 4GB black screens

Hi guys i have an RX 580 4GB Gigabyte model card. That gives me black screen and 100% fan spin anytime i try to run Furmark or anything that put's it under load.I have run a Polaris Memory test using arch Linux and it generated the following errors. Can someone please interpret what it is actually saying and if all the memory banks might be bad ? It uses Samsung Memory btw.Really appreciate it thanks.

I was able to run Tserver and it gave me a similar confusing result. Anyone of you may know what this means ? Was told it might be a dead Core.

as always on this cards u nust replace ALL ram chips
