    in standby does the main transformer run? If so does the transformer put out more voltage when the smps is in full operation?

    The transformer does not usually run. Big transformers have a minimum magnetising current required to operate properly, which means they are generally inefficient and this would consume too much power. Legal requirement in EU is now less than 0.5W in standby.

      If the power supply is like a computer SMPS, there is kind of a second, smaller power supply that runs the 5V stand by circuit. Its generally a very simple two transistor setup on a cheaper unit, or a discrete IC, that has a low current rating. The 5V stand by circuit has its own, small, transformer.
